Because she's shagging Tom Baker (They got married a year later (I think it was then?) Also the Doctor has a different lapel badge. Was it just this story Phil, or all season 17?

Indeed. They got married just after she left, and then separated soon after. Lalla Ward used to design her own costumes and they were all a bit unusual. But at least it was her picking them rather than being told to dress like a school girl.
City of Death is the first overseas location shoot too. Which is why there's so much random running around the streets.

It is, because it was a lost story which was unearthed in Hong Kong in the early 90s and rushed out onto video in a blaze of publicity. At which point everyone could evaluate for themselves how good it was instead of having to rely on the ancient memories of 60s fans telling them it was good. It's probably worth pointing out that 90% of of the DVDs you have are regarded as series classics; I don't think anyone could claim the series as a whole is as consistently good as the stories you've been watching Not *all* of the good stories by any means though.
